Utah mine, trucking company cited for driver's death

WASHINGTON (AP) -- Federal mining regulators cited the operator of Bear Canyon Mine in Emery County and a trucking company for the crushing death of a coal hauler.

David Whiting, 31, was trapped in December between the front bumper of his truck and a front-end loader.

The loader was on a steep grade trying to pull the truck out of a muddy ditch.

The Mine Safety and Health Administration said the loader's parking-brake system wasn't working and that the operator should have used wheel blocks.

The agency cited C.W. Mining for those problems and Trimac Transportation Services Inc. for failing to train Whiting in safety procedures.
