MOAB — Three people were killed in a head-on collision Sunday afternoon outside of Moab.

A Chevrolet Suburban was heading south on state Route 191, 25 miles north of Moab, when it entered a northbound lane about 12:15 p.m.. The Chevrolet struck a motor home pulling a GMC Canyon head on, Utah Highway Patrol said.

The driver and two passengers of the Chevrolet were killed in the crash. Two additional passengers in the Chevrolet and two occupants of the motor home were taken to a hospital for unspecified injuries, UHP said. The agency said it is investigating impairment as a contributing factor in the crash.

A fundraiser posted Monday morning identified the deceased as Derick Hurst, his wife Shay Hurst and their baby Skylar. A son was taken to a local hospital, and another was flown to a Salt Lake City hospital.

The GoFundMe* was made by a family friend and said the family was traveling home to Mancos, Colorado, from a softball tournament in Vernal.

"The family leaves behind five kids that will need support and care moving forward, as well as three funeral expenses. This is to help the family as much as possible in this tragic time," the fundraiser states.

Utah Highway Patrol said Derick Hurst was 31, Shay Hurst was 28, and the baby was only 1 month old, but the GoFundMe states Skylar was 3 months old.
