CHARLESTON, Wasatch County — Authorities identified a Heber City woman killed in a crash on U.S. 189 near Deer Creek Reservoir Monday evening.

Utah Highway Patrol officials said Heather Petersen, 23, died in the crash and Jose Francisco Lozoya Tellez, 37, of Heber City, was also critically injured in the crash. No update was given on his condition Tuesday.

Troopers responded to a report of a crash near Dock of the Bay in Deer Creek Reservoir, about two miles south of Charleston, around 5:15 p.m., UHP officials said.

Troopers said a Chevy Trailblazer, driven by the 37-year-old man, traveling north on the highway crossed into the southbound lanes and struck a Toyota Camry driven by Petersen. Investigators said the Camry swerved to the right to avoid the Trailblazer but the two vehicles collided “nearly head-on.”

Petersen died at the scene and Tellez was transported to a hospital with critical injuries.

Troopers were unsure Tuesday why the Trailblazer crossed into the southbound lanes. The cause of the crash remains under investigation.
