Brooke Walker and Andrew Adams reporting Some animal advocates are outraged after another serious error by a Utah Valley Animal Center. So far this year, six dogs have been euthanized by mistake.

The shelter takes full responsibility, but some say that's not enough.

Six dogs dead. All mistakes some people say could have been avoided.

The most recent death happened this week. A Provo family arrived to pick up their two dogs being held at the shelter while the family was out of town. But when they showed up, they learned one of the dogs had been put down by mistake.

The same thing happened to Barbi Carroll back in April.

Barbi Carroll, Dog killed: "If Peyki's death would have been taken seriously enough, their dog wouldn't have been put to sleep."

Carroll used to work at the shelter and was devastated when employees told her they accidentally eauthanized her 8 ½ year old lab, Peykos.

Barbi Carroll: "We don't have kids. We had two dogs. So, it's hard."

Now, she wants action to prevent this tragedy from happening again.

Barbi Carroll: "They're not being punished for it. The only people being punished are myself and the other owners going through it."

A shelter spokesperson told KSL improvements have been made and future changes are in the works.

He said the animal shelter is staffed with "good people who are trying hard. The lack of experience and lack of employees have led to these tragic mistakes."

A board made up of local city officials oversees the shelter. They are optimistic about the future.

John Borget/ Board of Directors: "We still feel we are heading in the right direction. We have made changes and will continue to make changes."

We want to point out, a previous criminal investigation cleared the shelter of any wrongdoing. The board will be meeting Monday night to discuss the issue.
