KSL 5 GOLF UTAH/Siegfried & Jensen "Putt for Cash" CONTEST

  1. Dates/Eligibility. The KSL 5 Golf Utah/ Siegfried & Jensen "Putt for Cash" Contest (the "Contest") begins March 27, 2010 and ends August 25, 2010. KSL 5 TV (collectively, the "Station") may change the dates and/or terms of the Contest without prior notice. Any material changes will be posted on www.ksl.com (the "Site") and studio5.ksl.com. The Contest is open to all legal residents of Utah, eighteen (18) years of age and older at the time of entry. There is no purchase necessary to enter or win. A purchase will not increase a contestant's chances of winning. The Contest is void where prohibited by law. Employees or agents of the Station, Bonneville International Corporation, Siegfried and Jensen, any entity associated with the administration or sponsorship or conduct of the Contest (collectively, the "Sponsors") or any other television or radio station that serves the Salt Lake City, Utah area, as well as members of the same household of any such employee or agents, are ineligible to participate.

  1. How to Enter. Beginning March 27, 2010 and continuing until August 19, 2010, contestants can enter the Contest by visiting the any participating Smith's Food and Drug and submitting a completed "KSL 5 Golf Utah/Siegfried & Jensen Putt for Cash Contest" official entry form. Alternatively, contestants may enter by sending a postcard with the contestant's name, address, age, telephone number, and the name of the Contest to KSL 5 TV, Broadcast House, 55 North 300 West, P. O. Box 1160, Salt Lake City, Utah 84110-1160. To be considered valid entries, postcards must be received by the Station on or before August 23, 2010. Limit one (1) entry per person, regardless of entry method. All entries become the sole and exclusive property of the Station.

  1. Selection of Winner. On Monday, March 29, 2010 followed by each Friday thereafter starting on April 2 and ending August 20, 2010, one (1) winning entry will be chosen at random from all eligible entries received each time. The Station will attempt to contact the person whose name is drawn using the telephone number and email address provided on the entry form. If the Station is unable to contact the person in a timely manner, the Station will discard the person's entry and select an alternate entry. Each Wednesday starting March 31, 2010 and ending on August 25, 2010 one (1) person will be given the opportunity to score a 50 foot putt at a preselected time to be held at Thanksgiving Point near Lehi, UT. If the person hits the 50 foot putt in one shot, he or she will win a Grand Prize of $10,000.

  1. Prize. The number of prizes awarded in the Contest is contingent on how many finalists shoot a hole in one at the designated "Putt for Cash" Golf Tournaments. Each Grand Prize is a $10,000 check provided by Siegfried & Jensen. The approximate retail value of each Grand Prize is $10,000. The winner will be responsible for any taxes, fees, gratuities or other charges that result from the receipt and/or use of the vehicle. The winner may be required to complete an IRS Form W-9 and will receive an IRS Form 1099-Misc. The winner will also lose their amateur golf status for one year. Additional restrictions may apply.

  1. Conditions/Restrictions. In exchange for the right to participate in the Contest, each contestant agrees to release and indemnify each of the Sponsors, and the officers, directors, agents, parent companies, subsidiaries and employees of the Sponsors, for, from and against any and all claims, demands and/or causes of action of any nature or kind whatsoever, whether presently known or unknown, foreseen or unforeseen, that arise out of the contestant's participation in the Contest, and, if the contestant wins the Van, the use of the Van. Entry into the Contest constitutes permission for each Sponsor and/or its respective designees to use the contestant's name, likeness, portrait, voice and/or biographical material for an unlimited period of time in order to advertise, promote or publicize the Sponsor and/or the products and services of any of its designees. As a condition of receipt of his/her prize, the winner will be required to produce identification satisfactory to the Station and sign an affidavit of eligibility and release (including a publicity release) as prepared by the Station. Anyone using fraudulent means to participate in and/or win the Contest will be subject to disqualification. The Sponsors are not responsible for technical, hardware, software or telephone or other transmission failures of any kind, lost or unavailable network connections or incomplete, garbled or delayed computer transmissions, regardless of cause. The Station reserves the right in its sole discretion, to cancel or suspend the Contest should a virus, bug or other problem beyond the control of the Station corrupt the administration, security or proper execution of the Contest.

  1. Rules. Copies of these rules are free and available at the Sites, at the Station's studios, Broadcast House, 55 North 300 West, P.O. Box 1160, Salt Lake City, Utah 84110-1160, during regular business hours or by sending a request, along with a self-addressed stamped envelope, to the Station at this address.
