Utah state officials weren't immediately available for comment.

SALT LAKE CITY (AP) — Utah officials say a clerk's mistake caused former Republican presidential nominee Mitt Romney to sign a form that would have registered him as an unaffiliated voter living at an outdated Utah address. The Salt Lake Tribune reports the mistake last month involved a pre-printed registration form Romney signed while obtaining a driver's license. Romney is being sent a new form, which an aide says he will fill out as a Republican.
