NORFOLK, Va. (AP) — A new website launched by an Eastern Virginia coalition allows people to create advance directives for health care.

The As You Wish website offers a downloadable form that can be used to create an individual advance directives plan. Options include a do-not-resuscitate directive and the designation of someone to make health care decisions in the event the person is incapable of doing so.

The website is part of the Advance Care Planning Coalition of Eastern Virginia's As You Wish program. The program's goal is to raise awareness of advance directives and to educate people about their value.

Coalition members include four agencies on aging and the Bon Secours, Chesapeake Regional Medical Center, Riverside Regional Medical Center and Sentara Healthcare hospital systems.
