KEARNS, Utah (AP) — Police have identified a driver who died after hitting another vehicle head-on and then crashing into a Kearns home.

KUTV in Salt Lake City reports (http://bit.ly/1STf9vJ) that Unified police said Sunday that 34-year-old Eric Douglas Forney, of Magna, could not be revived after the crash.

Authorities say Forney rear-ended a pickup truck with a man and woman inside. He then allegedly left the scene but the occupants of the car followed him. They say he hit poles and a parked car.

Police say Forney kept driving until he hit the home, destroying the garage. Forney suffered fatal injuries but no one else outside or in the home was hurt.

Unified police spokesman Manfred Lassig says evidence indicates alcohol was likely involved in the crash.
