HONOLULU (AP) — State health inspectors have fined a second Oahu restaurant for removing an inspection placard.
The Honolulu Star-Advertiser (http://bit.ly/1urPwqi) reports Ichiben restaurant, owned by 3 W Restaurant Group LLC in Waimalu, was fined $8,000 for removing a yellow "Conditional Pass" placard issued for health violations.
The restaurant can appeal the fine. A manager reached by phone Friday had no comment.
The Health Department says an inspector Dec. 6 found an inaccessible hand wash sink, improper sanitizing of dishes, improper food temperatures and cross-contamination of utensils touching raw poultry.
An inspector Dec. 29 noticed the placard was missing. It was replaced a day later.
The restaurant corrected the violations and now has a green placard.
The Health Department last year fined a restaurant at Ala Moana Center $11,000 for removing a placard.
