SARATOGA SPRINGS, Utah (AP) -- Police have released the identity of the man who died in a head-on collision on U-73.

Jay Rodney Syndergaard, 40, Tooele, died at the scene Saturday. The truck he was driving swerved into the path of an oncoming SUV. Syndergaard was partially ejected from his truck during the accident.

The driver of the SUV, Richard Barnum, 45, of Cedar Valley was treated and released Saturday from American Fork Hospital, along with four of his five children, Richard, 16; Anna, 11; Cody, 9; and Charles, 5.

Barnum's wife, Julie, remained in the hospital over the weekend, suffering from a bruised lung and broken clavicle, but was released Monday morning.

Daughter Samantha Barnum, 15, suffered a broken pelvis and was flown to Utah Valley Regional Medical Center, where she remained in stable condition.
