(KSL News) -- A Herriman day care center has lost its license.

Yesterday, the Utah Department of Health officially revoked the permit for Small Wonders Development Center.

Salt Lake County sheriff's deputies raided the center three weeks ago, after a tip from a former employee.

Officers arrested director Michaele Meier and two employees.

Meier scheduled an appointment to appeal a revocation of license notice, but didn't show up.

The center was found in violation of several rules, including failing to pass background checks and using inappropriate discipline.
