WELLSVILLE, Cache County — One woman died and two men were injured after a vehicle traveling in the wrong direction collided head-on with another vehicle on U.S. Highway 91, authorities said.
Emergency crews responded to a report of the crash about 2:13 p.m., according to Utah Highway Patrol officials. The incident took place near mile marker 11 of the highway, at the Dry Lake area of Sardine Canyon, UHP said.
Utah Highway Patrol Lt. Lee Perry said one person was transported via medical helicopter to a hospital and two others were transported by ambulance.
Witnesses at the scene told UHP troopers that a 2014 Chevy Silverado crossed into the median near the Sardine Canyon summit and was traveling north in a southbound travel lane, according to the agency.
The Silverado crashed into a 2014 Toyota Sienna that was traveling south, UHP said. The Sienna ended up on top of the median, which required emergency officials to close the southbound travel lanes and one northbound lane for several hours, according to UHP.
An 87-year-old woman, who was a passenger in the Silverado, died in the crash, UHP officials said in a news release. The driver of the Silverado, an 87-year-old man, was taken to Logan Regional Hospital in good condition.
The driver of the Sienna, a 32-year-old man, had to be extricated but was taken to the hospital in good condition, according to UHP.
Authorities did not identify any of the people involved in the crash.
UHP initially reported on Twitter that the woman died after the crash, but later reported that the woman "regained a pulse" and was transported in a medical helicopter in extremely critical condition. Perry confirmed that she died while on the way to Ogden Regional Medical Center.
Perry added preliminary evidence indicates one of the drivers may have been traveling in the wrong direction for at least 1/2 mile.
Officials are investigating fatigue or a medical condition as possible causes of the crash, according to UHP.
Utah Department of Transportation officials said southbound lanes of the U.S. 91 were closed slightly over an hour eight miles southeast of Logan. As of 4:15 p.m., the road had reopened, according to Cache County emergency dispatchers.
