PROVO — On Dec. 18, 2015, Jake Toolson dropped in a 3-pointer in BYU’s 98-85 win over Central Michigan.
Unknown at the time, it would be his last bucket in a Cougar uniform.
Toolson, who took a medical leave of absence just a few days after the game, will transfer from BYU, KSL has confirmed. ESPN’s Jeff Goodman first reported the transfer Wednesday afternoon.

Toolson’s destination is currently unknown. Prior to enrolling at BYU, He was also recruited by Arizona State, Boston College, Utah State and Saint Mary's.
The native of Gilbert, Arizona, appeared in 10 games in 2015-16 for the Cougars, including five starts, averaging 3.9 points, 1.2 assists and 1.6 rebounds while helping his squad to a 7-3 record.
As a freshman, Toolson, who is the nephew of former BYU All-American Danny Ainge, averaged 2.3 points, 1.4 rebounds and 0.5 assists in 7.8 minutes across 26 games off the bench. He totaled a career-high 15 points, seven rebounds, four assists and four steals Nov. 19, 2014, in a win over Division III Southern Virginia.
Because Toolson played in only 10 games as a sophomore and left for medical reasons, he could be a candidate for a medical redshirt to gain back his 2015-16 season. The appeal would be contingent on a ruling by the NCAA, but it would be a similar situation to the one gained by teammate Nate Austin, who earned an extra season after playing in less than 10 games before suffering a season-ending hamstring injury against Utah two years ago.
Since announcing his medical leave Dec. 21, Toolson was also removed from BYU basketball's active roster. He was present at the Cougars' annual season-ending banquet Monday night in downtown Provo.
After the 2015-16 season, BYU was set to lose starters Kyle Collinsworth and Chase Fischer, as well as Nate Austin, to graduation, while Zac Seljaas will serve a two-year mission in Iowa. The Cougars added returning missionaries Eric Mika, T.J. Haws, Payton Dastrup and freshmen Steven Beo and Yoeli Childs.
The quintet of newcomers joins returning scholarship players Kyle Davis, Nick Emery, Jakob Hartsock, Braiden Shaw, Corbin Kaufusi, Jordan Chatman, Jamal Aytes and Elijah Bryant, as well as Davin Guinn and Alan Hamson.
