To enter, visit either www.ksl.com or http://studio5.ksl.com, log on to the site and submit your story (in 500 words or less) and a photo of you and your smile. By submitting your entry you agree to the contest rules and to receive further information from Dr. Rod Gleave. (See Below)

1.    The Beautiful Smile Makeover by Dr. Rod Gleave is open to residents of Utah only. Contestants must be 21 years of age or older. Employees of Dr. Rod Gleave and their immediate families and the sponsors are not eligible. No purchase necessary to enter or win, nor will a purchase improve one's chance of winning. Odds of winning will depend on the number of eligible applications received.
2.    Contestants who wish to enter the Beautiful Smile Makeover by Dr. Rod Gleave may do so beginning Sept. 6, 2010 by logging on to www.ksl.com or http://studio5.ksl.com , submitting an essay (500 words or less) on how the Beautiful Smile Makeover by Dr. Rod Gleave will change their life, and submitting a recent photo of their smile. Entry to the Beautiful Smile Makeover by Dr. Rod Gleave will be via the ksl.com web site only. Only one entry per person will be accepted. Entries must be received via online submission by 11:59PM EST Sept. 27, 2010. Each entry must be complete to be accepted. Dr. Rod Gleave shall not assume any responsibility for entries that are not received. All materials submitted become property of the office of Rod Gleave DMD and FUEL Marketing and will not be returned and may be used for promotional purposes.
3.    Dr. Rod Gleave DMD retains the right to reject any entry form that he believes to be incomplete, illegible or inappropriate for any reason. The applicant must be in excellent mental and physical health and cannot be pregnant, or planning on becoming pregnant in the next six months. Contestant must be up to date with their basic dental care (within the past 12 months).
4.    All eligible finalists will be mandatorily screened by Dr. Rod Gleave and/or his representatives at the office of Rod Gleave DMD  located at 622 East 4500 South #102 Salt Lake City, UT 84107 in order to determine contest eligibility and the level of dentistry that is necessary. Dr. Rod Gleave has the right to remove eligibility of any entrant based on findings during the screening process.
5.    Contest judges reserve the right to decide all features of the awards offered while winning applicant has the right to refuse any or all of the procedures presented. All procedures and any incidences of complications will be discussed prior to services.
6.    Winner will be notified by phone, e-mail, and/or mail no later than Sept. 28, 2010 of their selection and must complete a personal interview to determine eligibility and will also be required to sign the office of Rod Gleave DMD's affidavit of eligibility in order to collect the prize. This affidavit must be returned within one week of date of notification or an alternate winner may be selected.
7.    By participating in the Beautiful Smile Makeover by Dr. Rod Gleave, entrants agree to hold Sponsors, their respective directors, officers, employees and assigns harmless against any and all claims and liability arising out of the award or use of the prize.
8.    Prize: One winner of the Beautiful Smile Makeover by Dr. Rod Gleave will receive a dental makeover from the office of Rod Gleave DMD. The maximum value of the makeover will be $40,000 in combined products and dental services, estimated by Dr. Rod Gleave and his staff. There is no cash value to the prize and prize is non-transferable and non-assignable.
9.    Finalists: All finalists will receive 10% off the cost of their redesigned smile from Dr. Rod Gleave. All entrants in the contest will receive a Free Custom Whitening Kit with a paid checkup from Dr. Rod Gleave's staff. This offer expires 60 days after the close of the contest.
10.    All entrants agree that sponsors may use their name, photographs, videos, contest entry and statements concerning their Beautiful Smile Makeover by Dr. Rod Gleave entry, without compensation, for purposes of advertising and promotion and also grants all rights to edit or modify and to publish and copyright it. By participating in the Beautiful Smile Makeover by Dr. Rod Gleave, entrants agree to hold sponsors, their respective directors and employees harmless against any and all claims and liability arising out of the award or use of the prize.
11.    Winner must be available for several days for the smile makeover process and must make all appointments and/or advertising appearances. Missing appointments or appearances could jeopardize the completion of the dental makeover and could result in disqualification. If the chosen participant is disqualified for any reason, an alternate participant will be chosen to complete the Beautiful Smile Makeover by Dr. Rod Gleave. The alternate will be chosen from the same application process.
12.    All decisions of Dr. Rod Gleave are final and absolute and not subject to inquiry or appeal.
13.    All federal, state and local laws and regulations apply. Void where prohibited or restricted by law. Taxes, if applicable, are the winner’s responsibility. All information submitted by entrants as part of this Contest may be used by Sponsor to communicate with entrant for Sponsor’s or third party marketing purposes. No purchase necessary. Void where prohibited.
14.    BY ACCEPTING PRIZE, WINNER ACCEPTS THE INHERENTLY DANGEROUS NATURE AND RISK IN DENTAL TREATMENT AND RECEIVING DENTAL CARE, INCLUDING PERSONAL INJURY AND/OR DEATH, ACKNOWLEDGES THAT HIS/HER PARTICIPATION IN THE DENTAL PROCEDURE IS VOLUNTARY, ACKNOWLEDGES THAT THERE ARE NATURAL FACTORS AND OCCURRENCES WHICH MAY IMPACT ON OR AFFECT THE SAFETY OF THE ACTIVITIES HE/SHE IS PARTICIPATING IN AND HE/SHE ASSUMES THE RISK OF SUCH FACTORS AND OCCURRENCES AND AGREES THAT PROMOTION PARTIES SHALL NOT BE IN ANY WAY RESPONSIBLE FOR ANY RESULTING INJURY AND/OR DEATH.
15.    General Conditions: By entering this Contest, you accept and agree to abide by the Rules of this Contest and the decisions of the Sponsor in connection with the Contest, which are final on all matters relating to the Contest. Entrants further represent and warrant that: [1] they own all right and title to photos submitted; it does not infringe upon the copyrights, trademarks, rights of privacy, publicity or other intellectual property or other rights of any person or entity; they have obtained prior permission from the photographer, if photographed by someone other than entrant; and they hereby agree to defend, indemnify and hold harmless Sponsor, its directors, officers, employees, representative and agents from any and all liability arising from any claims to the contrary; [2] submission of the Entry into the Contest grants Sponsor and its assigns an unlimited, worldwide, perpetual license and right to publish and use entrant's name, address (city and state only), voice, likeness, photographs and statements about the Contest for any programming, publicity, advertising and promotional purposes, in any and all media, without limitation, and without additional compensation, except where prohibited by law; [3] by entering this Contest, entrants agree to release Sponsor, its directors, officers, employees, representatives and agents from any and all liability arising out of the participation in this Contest, including but not limited to claims for defamation, invasion of privacy or portrayal in a false light (whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory.
16.    Winners Conditions: By accepting the Prize, winner further agrees, represents and warrants to: [1] release Sponsor, KSL,  and their respective directors, officers, employees, representatives and agents from any and all liability arising out of winner's acceptance of the Prize, including without limitation, personal injury, death, and property damage; [2] agrees that his/her photograph will be deemed a Work Made For Hire under the Copyright laws of the United States, but if it cannot be so deemed, irrevocably assigns and transfers to Sponsor all of his/her right, title and interest in and to his/her Entry, including but not limited to any and all copyright and trademark rights which he/she may have, in the United States and worldwide; [3] waiver in favor of Sponsor, all rights of "Droit Moral" or "Moral Rights of Authors" or any similar rights or principles of law that winner may now or later have to their photograph; [4] upon request of Sponsor, winner shall execute and deliver such additional instrument of assignment, as may be solely deemed by Sponsor, reasonably necessary to establish the ownership of record of the right, title, interest and Moral Rights of Author in and to the Entry, Publicity Materials and Updates, as defined herein and hereunder.  Failure to request the said assignment(s) shall not constitute a waiver of Sponsor's rights and Sponsor at a later time may request the assignment(s). 
17.    Additionally, as a condition of claiming the Prize, winner agrees, acknowledges and understands that: [1] he/she must allow Sponsor to take before and after photographs of winner's teeth, allow Sponsor's film crew to film winner's dental makeover, which includes but is not limited to filming winner's dental treatments and provide Sponsor with on camera interview(s) regarding the Contest ("Publicity Materials"); [2] all dental work constituting the Prize must commence and be completed by Dentist within sixty (60) days from the date in which the winner is notified; Dentist shall solely, and with no input from Sponsor, devise a treatment plan for winner and shall perform any work consented to in the Dentist's treatment plan.
18.    Refusal or inability of the winner to comply with any of the above conditions may result in disqualification as a winner or forfeiture of the Prize.  Failure to keep scheduled dental appointments and/or have dental work completed within sixty days (60) from the date in which the winner is notified may result in forfeiture of the unused remaining value of the Prize.  Winner is responsible for any travel and/or lodging costs associated with claiming the Prize, including the value of any time off from work or otherwise in connection with claiming the Prize.
19.    Winner Updates: During the course of winner's smile makeover, Sponsor may ask winner to chronicle his/her smile transformation by providing Sponsor with updates subject to the blogging guidelines provided by Sponsor ("Updates"). Updates are defined as a video blog ("Blog") and/or photos, twitter™ "tweets," facebook® status updates or other communication that the winner submits to Sponsor. All Updates must comply with these Official Rules. Any and all Updates provided by the winner shall be deemed Work(s) Made for Hire under the Copyright laws of the United States and shall become the property of Sponsor upon submission. Winner, if requested, shall irrevocably transfer and assign to Sponsor all his/her right, title and interests in and to his/her Updates. Winner understands and agrees that any Updates provided to Sponsor shall be deemed to have been provided on a gratuitous basis and winner is not entitled to any additional compensation, except for the Prize as defined herein.
20.    Notice: Any attempt by an individual to deliberately damage any website or undermine the legitimate operation of this Contest may be a violation of civil laws.  Should such an attempt be made, Sponsor reserves the right to seek damages from any such individual to the fullest extent permitted by law.
21.    Disputes: These Rules shall be governed and construed in accordance with the laws of the State of Utah. Any disputes arising out of these Rules shall be resolved through the American Arbitration Association in Salt Lake City, Utah, and under its then existing rules governing arbitration of commercial disputes.  Any award made by an arbitrator shall be binding upon the parties and may be entered into judgment in a court of competent jurisdiction. Any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Contest, but shall not include   attorneys' fees. Entrant hereby waives all rights to claim punitive, incidental, and consequential damages and/or any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. If any provision of these Rules  is deemed to be invalid or unenforceable, in whole or in part, such provision will be deemed to be modified or restricted to the extent and in the manner necessary to render the same valid and enforceable and these Rules will be construed and enforced to the maximum extent permitted by law as if such provision had been originally incorporated herein as so modified or restricted, or as if such provision had not been originally incorporated herein, as the case may be.
