The Quality/HIS Specialist - is responsible for assisting in the planning, organizing and facilitating the Hospice Item Set (HIS) and other quality instruments, assessments, and data entry. This position is responsible for the data collection and organization of the quality improvement organization and is accountable for the monitoring, implementation and evaluation of HIS processes and programs to meet regulatory, accreditation and standards of practice.
Communicate and coordinate with hospice and homecare locations to ensure timely submission of location data. Adhere to all state, federal, and organizational timelines related to entry and submission of quality measures. Assist in planning, organizing and facilitating the gathering and organization of HIS data from multiple locations. Provide expertise and coordination of HIS collection and aggregation activities utilizing continuous quality improvement. Support agency activities related to HIS data to ensure timely submission. Compile, aggregate, and assist with submission of HIS data in the following areas: patient care, safety, risk management, chart/documentation audits, infection control, outcomes, DEYTA, QAPI Navigator, and customer satisfaction. Collaborate with the location leadership and the Corporate Billing Department in trending HIS needs. Provide, trend, and track staff education as directed. Maintain HIS databases and distribute according to Bristol Policy and processes.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Must be detailed oriented and have strong clinical skills.
