    Job Description

    Registered Nurse / Case Manager Immediate need. Full-time RN/Case Manager for a healthy and accredited Home Health and Hospice provider. Abundant Life is looking for a patient focused, caring candidate to work from their Ogden location as an RN, providing exceptional care to patients in their home environment. Care includes comfort and aid to terminally ill patients on Hospice service, and also to those needing home-based rehabilitative services through our Home Health service. You will have the opportunity to utilize your clinical skills in providing for the medical, personal, and physical needs of the patient, while demonstrating excellent communication skills in the timely documentation and coordination of a comprehensive patient care plan of care. As a RN/Case Manager you will oversee and coordinate the work of other team members in providing patient care by taking complete ownership of the care of your patients. This is an important and exceptional opportunity for someone truly wanting a full-time, responsible position.

    Relocation costs may be covered.


    Requirements: * RN License. * CPR certification. * Home Health experience required, with additional Hospice experience desirable. * 2-3 years of successful RN/Case Manager experience. * Exceptional communication skills are a must. * Valid Utah drivers license, with insured and reliable transportation. * Highly developed organization skills, and attention to detail. * Ability to fully evaluate, plan, coordinate, and monitor all aspects of patient care. * Compliance with accepted professional standards and practices.
