Quality Solace Services Lifecare & Hospice Agency is currently hiring Hospice and Home Health nurses. Home Health Care Registered Nurse ??? RN (Full Time / Part-Time) Hospice Care Registered Nurse- RN (Full Time / Part-Time) Hospice & Home Health Case Mix Registered Nurse - RN (Full Time / Part-Time) Pediatric Hospice Registered Nurse - RN (Full Time / Part-Time) We are also looking for Certified Nursing Assistants, Occupational Therapists, Occupational Therapy Assistants, Physical Therapists and Physical Therapy Assistants, Respiratory Therapists, and Pediatric Hospice Nurse Practitioners with a MINIMUM of 2 years experience in Home Health or Hospice care.

Responsibilities: As a Registered Nurse, you will visit multiple patients??? homes each day and provide a full range of personalized care, which includes pain management, wound care, IV infusion, and post-operative instruction. Part of your job is to work with patients and strive to meet the patient's needs. Seeing a patient achieve physical and emotional comfort during a distressing time so that they can live life as fully as possible is just one of the many rewards of this position. Your specific duties as a Registered Nurse will include: Case management as per Utah state rules, Federal regulations, and CHAP Standards of Excellence for all cases involving nursing and therapy services Initial evaluation visits as necessary as well as regular patient need(s) re-evaluations Providing nursing services, treatments and diagnostic procedures requiring specialized skills and assuming full responsibility for each of your patients Regular participation in all corporate in-service programs, as well as training, and teaching other nursing personnel Development and implementation of treatment plans for each patient under the direction of the attending physician and or hospice medical director Observing and reporting symptoms, reaction to treatments, drugs, and changes in the patient???s physical or emotional condition Initiating symptom management, preventative and rehabilitative nursing procedures as appropriate for the patient???s care and safety Maintaining clinical and progress notes for each patient receiving care and providing progress reports to their physician(s) Coordination of care and services for patients as needed Counseling the patient and their family in meeting nursing and related needs Providing supervision of home health aide services and LPN employees and preparing aide care plans with written instructions for cares provided by home health aides Maintaining confidentiality of patient and agency matters Completing required documentation in a timely manner ??? Electronic equipment provided to the Registered Nurse for the agency's point-of-care, EDI, paperless charting system. Qualifications: As a optimal candidate for the Registered Nurse positions, you must be dependable, professional and energetic with a positive and empathetic personality. You will also need to have strong organizational, assessment and problem-solving abilities. It is also important that you display excellent communication and interpersonal skills as well as the ability to interact well with patients, families, physicians and staff. Specific qualifications for the Registered Nurse position include: Graduate of an accredited school of nursing Valid and current RN license in Utah state. Mandatory 2 years minimum nursing experience in either home health, hospice, or palliative/hospice pediatric care Solid supervisory nursing skills Strong sense of initiative and urgency in addressing unexpected patient crises Commitment to regulatory compliance and to submitting all documentation in a timely fashion Demonstrated knowledge regarding exposure to body fluids and chemicals, community nursing infection control as well as to some hazardous equipment Ability to see and hear adequately to respond to auditory and visual requests Evidence of adequate health status to perform duties Ability to pass a background screen Ability to pass a drug/alcohol screening Valid and Current Utah State Driver's License, dependable automobile, valid and current auto insurance.
