Job Description
Medical social worker needed for a skilled nursing facility. Primary responsibilities will include discharge planning, completing psychosocial assessments, counseling and providing resources to residents and family to assist in discharge plans. Will schedule and carry out interdisciplinary team meetings with family and residents. Looking for someone with experience in the medical field and the ability to work in a fast paced medical setting. SSW license required. Competitive pay.
Requirements
Minimum of a bachelors degree in social service field.
