Job Description
Summit Home Health & Hospice has an immediate opening for a full time, benefited, Utah County Hospice RN Case Mgr. This position requires availability at least Monday-Friday, 8-5. Duties include admissions, visits, etc. Coverage of entire county. Ability to assess and document patients for eligibility. Management of terminal patients and support of patients and families. Working as a member of the interdisciplinary team.
Requirements
Applicants must be a currently licensed RN in the state of Utah in good standing. Must have current CPR certification. Must have reliable transportation and cell phone. Only apply if you have previous hospice experience.
