EAGLE MOUNTAIN — A woman was killed Monday afternoon after several concrete forms slipped and landed on her during a delivery.

The accident occurred around 11 a.m. at an Eagle Mountain storage facility on Stagecoach Drive, according to Utah County Sheriff’s Sgt. Spencer Cannon. The victim was identified as Cynthia "Cindy" Weeks, 49, of South Jordan. Weeks was helping her husband deliver 2-feet-by-8-feet concrete forms on a flatbed trailer to the storage area.

During the delivery, Weeks was standing next to the trailer while the forms were being unloaded from the opposite side, Cannon said. The load slipped and 30 concrete forms landed on Weeks. Cannon said the concrete forms weigh about 70 pounds each.

Witnesses called 911 and began to remove the forms to free the victim. A Life Flight medical helicopter responded to the accident, but Weeks was pronounced dead at the scene.
